PLACEMENT SUMMARY – MARKETING, DIGITAL MEDIA & GRAPHIC DESIGN PROJECT COORDINATOR: # Positions available to work individually - 2 We are a small, productive team focused on providing high-value programs, services, and learning opportunities to our members and stakeholders. We have a robust strategic plan and each team member knows how their work impacts the organization’s priorities. Our culture is open and collaborative; everyone pitches in and innovation is encouraged. We are seeking a practicum student to assist with our marketing, digital media, and social media strategies. The selected individual will focus primarily on developing and launching a provincial Compassionate Communities website, interactive tool kit, and social media campaigns. KEY RESPONSIBILITIES: · Conceptualize creative ideas to design website. · Develop content for all digital and social media channels. · Website design learning Advantage CSP · Build meaningful connections and encourage stakeholders to act and support HPCO. · Develop and execute a digital outreach strategy that engages hospices, volunteers, and other clients. · Build and execute social media strategy and engagement campaigns that attract stakeholders. · Maintain presence across all relevant social media platforms, including writing engaging content and relevant visual imagery. · Own and maintain site analytics, metrics, campaign reporting, including recommendations for optimizing website and social media engagement. · Manage and update webpage content with active stories and calls-to-action. · Act as primary contact with HPCO’s technical partners to ensure ongoing website functionality and maintenance according to service agreements with partners and suppliers. · Other duties as assigned. COMPASSIONATE COMMUNITIES(Virtual Opportunity)

COMPASSIONATE COMMUNITIES (CC): : A Compassionate Community is a community of people who feel empowered to engage with and increase their understanding about the experiences of those living with a serious illness, caregiving, dying, grieving and those who are isolated, marginalized or vulnerable. Community members take an active role in caring for people and to assist those to live comfortably in their homes. Community members also connect people to supports, raise awareness about health, well-being, and end of life issues; and develop the capacity of others in the community. Compassionate Communities compliment health care interventions by paying attention to prevention, detection, harm reduction, and early intervention. Compassionate Communities provide a route map for care in neighbourhoods and alter social environments to improve outcomes PLACEMENT SUMMARY – COMPASSIONATE COMMUITIES PROJECT COORDINATOR: HPCO has established a Community of Practice (CoP) network comprised of approximately 25 local Compassionate Communities (CC) initiatives across Ontario. During this placement, students will have the opportunity to work with CoP project members to organize, consolidate and share evidence and knowledge to support CC uptake and adoption; provide mentorship and coordination amongst to CC CoP project members to advance provincial efforts; conduct interviews and collect data and evidence related to CC evaluation and population health impacts; contribute to the development of a provincial tool-kit to support community adoption of the CC model; and provide project management expertise to support the overall work of the CoP. This role will provide opportunities to work closely with experts in program evaluation and leads of the Compassionate Community movement across Ontario. KEY RESPONSIBILITIES: § Coordination and communication with project collaborators (i.e. project meetings, organizing documents, communicating key messages) § Allocation and reminders of tasks to project collaborators § Updating and revising the Project Charter along with project progression § Summarize current CC related activities and care models § Develop strategic partnerships with key stakeholders § Provide education, training and knowledge dissemination on CC-related activities and programs to potential new adopters § Innovation support for new CC-related ideas/initiatives in early adopter sites, and to measure and publish results § Results measurement and reporting § Public communications § Evaluation § Provincial implementation planning § Support the launch and updating of a public CC website and community tracker sites for all mature CC sites

HEALTH CARE CONSENT ADVANCE CARE PLANNING (HCC ACP)(Virtual Opportunity)

HEALTH CARE CONSENT ADVANCE CARE PLANNING (HCC ACP) : HPCO has led the Health Care Consent Advance Care Planning (HCC ACP) Ontario strategy since 2011. As such, HPCO has recently been mandated in the Ontario Palliative Care Network Annual Workshop to continue to develop and update materials for patients, caregivers, substitute decision makers, and clinical health care providers to engage in Advance Care Planning Conversations, Goals of Care Discussions and Health Care Consent in alignment with Ontario’s legal landscape, and the Health Quality Ontario (Ontario Health) Palliative Care Quality Standards. PLACEMENT SUMMARY – HCC ACP PROJECT COORDINATOR: HPCO accomplishes this goal through a broad and expert HCC ACP Community of Practice (CoP). It consists of a Leadership Advisory Team, Clinical Advisory Team, Regional Champions, Organizational Champions, General Membership, Lead Trainer Program, and several working groups including a cross sector of provincial associations, a research working group, and a pediatric interest group. The CoP utilizes the Speak Up Ontario Website as its way to communicate Ontario compliant content, resources, education, webinars, tools and materials to Health Service Providers, Non-Health Care Professionals as well as Individuals and Families. It is the number one provincial source for information about HCC ACP in Ontario. During this placement, the student will: · help to coordinate the refresh plan of the Speak Up Ontario website to improve user experience and user interface for clinicians, non-health care professionals and individuals and families. · have the opportunity to work with staff members to prepare communication summaries of their HPCO HCC ACP work for inclusion in the HPCO newsletter and other communication venues. · have the opportunity to work with CoP members to prepare communication summaries and profile their HCC ACP work in the community for inclusion in the HPCO newsletter and other communication venues. · help to support the public facing promotion of ACP through social media, web, awareness ads, including marketing and branding strategies. · Additional tasks or initiatives within this placement will be added based on the student’s skills and interests.
