The focus of this course is projects on population health management in healthcare settings: patient identification and outreach, screening and risk-stratification, stepped-care treatment to target, follow-up and evaluation.

Students are doctoral students with an existing management degree enrolled in either a clinical concentration (e.g., social workers, counselors, nurses, occupational therapists, etc.) or management concentration (M.B.A, M.H.A., M.P.H, etc.).

The student written assignments require they identify a population for the course, conduct a literature review based on a defined intervention, then design and evaluation of a population health program, including the program costs and anticipated return on investment (ROI).

This course and students are well suited for Riipen partner companies that include population health management programs.

Students work with partner Riipen companies as individuals or in small groups based on interest. This is an 8 week intensive course.

Expected outcomes and deliverables

The core deliverables for students are three assignments:

Assignment 1: Population health program problem focus, literature review, problem statement, approach, research questions and hypotheses.

Assignment 2: Population health program design or methods; patient identification, engagement and enrollment, screening and risk stratification, stepped care treat to target interventions, follow-up, and evaluation. A return on investment (ROI) is included in the evaluation section.

Assignment 3: A short video pitch on the population health management program.

Students are encouraged to negotiate with their Riipen business partners how to adapt their classroom assignment to reports suitable for use in the company.

Project Examples

The course includes a discussion of current approaches, includes predictive modelling methods for defining populations; interventions focusing on stepped-care approaches; engagement strategies, interventions, measurement, and follow-up.

Based on the learning outcomes for the course, examples of potential projects include:

  • Create a population-based integrated health management program employing the basic concepts of epidemiology for an integrated health setting.
  • Consider specific evidence-based strategies and interventions for multiple aspects of population health management, including disease management, employment wellness programs, and screening and assessment for disease which meets the population health management component of the triple aim.
  • Design and evaluate the use of Health Information Technology for a specific population health management program.
  • Critically evaluate population health management research studies.
  • Create a proposal for a population health management project (inclusive of health CO3, assessments, outcomes measures, evidence-based treatments, and ROI).
  • Assess the role of population health management in health care reform, as employed CO1 by the primary care medical home(s), and accountable care organization(s).
